Savannah's Playhouse Waiver and Release of Liability
Waiver must be completed by a parent or guardian who is 18 years or older.
- Acknowledgment of Risks I, the undersigned participant (or parent/guardian if the participant is a minor), acknowledge and understand that participation in gaming, virtual gaming, fun zone activities, parties/events, play activities, bouncy houses, balloon houses, jumping, and the consumption of food at Savannah's Playhouse Gaming (hereinafter referred to as “the Facility”) involves inherent risks. These risks include, but are not limited to, physical injury, illness, death, mental or emotional distress, and financial loss.
- Assumption of Risks I voluntarily assume full responsibility for any risks of loss, property damage, or personal injury, including death, that may be sustained by me or my minor child, or any loss or damage to property owned by me or my minor child, as a result of participating in activities at the Facility, whether caused by the negligence of the Facility or otherwise.
- Release and Waiver of Liability I hereby release, waive, discharge, and covenant not to sue Savannah's Playhouse Gaming, its owners, directors, officers, employees, agents, and representatives (hereinafter referred to as “the Released Parties”) from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me or my minor child, or to any property belonging to me or my minor child, while participating in activities or consuming food at the Facility, whether caused by the negligence of the Released Parties or otherwise.
- Indemnification I agree to indemnify and hold harmless the Released Parties from any loss, liability, damage, or costs, including court costs and attorneys’ fees, that they may incur due to my participation or my minor child's participation in activities at the Facility, whether caused by the negligence of the Released Parties or otherwise.
- Third-Party Actions I acknowledge that the Facility cannot control the actions of other participants or customers at the Facility. I release the Released Parties from any liability for any injury, sickness, death, mental or emotional distress, or loss caused by the actions of another party or customer.
- Personal Belongings I understand and agree that the Facility is not responsible for any damage to, or loss or theft of, any personal belongings, including but not limited to, electronics, phones, computers, vehicles, bikes, and other items, during or after my time at the Facility.
- Medical Authorization In the event of an emergency, I authorize the Facility to secure from any accredited hospital, clinic, or physician any treatment deemed necessary for my immediate care, and I agree that I will be responsible for payment of any and all medical services rendered.
- Food Consumption I acknowledge that the Facility provides food and beverages, and I understand that consuming such items involves risks, including but not limited to, foodborne illnesses, allergic reactions, and other health-related issues. I assume all risks associated with consuming food and beverages provided by the Facility and release the Released Parties from any liability arising from such consumption.
- Participation in Parties/Events and Play Activities I understand and acknowledge that participation in parties/events, play activities, bouncy houses, balloon houses, and jumping at the Facility involves risks. I voluntarily assume all risks associated with such participation and release the Released Parties from any liability for any injury, illness, death, mental or emotional distress, or financial loss that may result from participating in these activities.
- Governing Law This Waiver and Release shall be governed by and construed in accordance with the laws of the State of [Your State].
- Severability If any term or provision of this Waiver and Release is found to be unenforceable or invalid, that term or provision shall be severed from this agreement and the remaining terms and provisions shall remain in full force and effect.
